Replace Lost Or Stolen Passport

Learn how to report your passport lost or stolen. If by chance your passport was lost, or someone has stolen it, it is recommended that you report is immediately to protect yourself from identify theft.

You have the option to replace lost or stolen passport now or later. Once it is reported, they will invalidate the current passport, which means you will not be able to use it for international travel.

How to report lost or stolen passport

Below are 3 options to let them know your passport was lost or stolen. Please select the one that is most convenient to you.

Online

You can report through their online system. Click here and navigate to the Report Lost or Stolen tab.

By mail

If you prefer, you can report it through the mail by downloading, completing and signing Form DS-64 and then mail it back to the address listed on the form.

By phone

You can report over the phone by calling them toll-free at 877-487-2778 (TTY 888-874-7793). If you are planning to apply for a new passport, you will then need to fill out Form DS-64 and turn it in with your application.

How to replace lost or stolen passport

In order to replace your passport, you will be required to submit Form DS-11 in person. Use the table below to determine where to submit your form.

If Then
If you are planning to traval within the next 3 weeks or less and are in the US Make an appointment to apply in person at a passport agency or center to replace your passport
If you are not planning to travel within the next 3 weeks or less and are inside the US Make an appointment (if required) to apply at an authorized passport acceptance facility to replace your passport
You are outside the US You will need to contact a US embassy or consulate near you to replace your passport.

If you come across someones lost US passport, then please mail it in a sturdy envelope to:

Cancelling passport for deceased relative

If you have a relative who has died, you can turn their passport in for cancellation. Once they cancel it, they will return it back to you. Please send the following:

  • The valid passport
  • A certified copy of the death certificate
  • A letter requesting the cancelation and return (or destruction) of the passport
